Advance Health Care Directives are written, legal instructions that enable you to express how you want to be treated if you are seriously ill and unable to speak for yourself. A Living Will describes the type and extent of medical treatment that you desire for end-of-life medical care. A Durable Power of Attorney for Health Care appoints a trusted person to make medical decisions for you if you cannot make them for yourself. Both of these documents can be obtained from the Illinois Department on Aging by calling 1-800-252-8966. The Five Wishes s a detailed document that includes personal, emotional, medical and spiritual needs. You can get this document by calling Aging with Dignity at 1-800-594-7437.
